Terms of Service
Effective Date: October 23, 2026
These Terms of Service (“Terms”) govern your access to and use of the website operated by Veloryn Health, LLC (“Veloryn Health,” “we,” “us,” or “our”), including velorynhealth.com, related forms, appointment scheduling, payment services, administrative support, and telehealth coordination services.
By accessing the website, creating an account, submitting a form, booking an appointment, enrolling in a program, or purchasing a service, you agree to be bound by these Terms.
1. About Veloryn Health
Veloryn Health provides administrative, technology, scheduling, payment, vendor-management, customer support, and telehealth coordination services.
Veloryn Health does not practice medicine and does not independently make medical decisions unless it is specifically identified as a licensed healthcare provider under applicable law.
Clinical care is provided by licensed healthcare professionals and the applicable treating clinical practice. All medical decisions, including eligibility for treatment, diagnosis, prescriptions, laboratory orders, medication selection, dosage, and continuation of treatment, are made solely by the treating licensed provider.
2. Acceptance of These Terms
By using this website or our services, you confirm that:
- You have read and understood these Terms
- You agree to comply with these Terms and all applicable laws
- You are legally able to enter into a binding agreement
- The information you provide is accurate, complete, and current
If you do not agree to these Terms, you should not use the website or submit information through our services.
3. Eligibility for Services
Access to healthcare services may depend on several factors, including:
- Your age and legal capacity
- Your physical location at the time of treatment
- The state or jurisdiction in which the provider is licensed
- Your medical history and current health condition
- Identity and location verification
- Clinical appropriateness and patient safety
- Availability of providers, pharmacies, laboratories, and medications
- Compliance with intake, consent, monitoring, and follow-up requirements
Submitting an intake form, making a payment, or booking an appointment does not guarantee that you will be accepted for care.
4. No Emergency Services
This website and our routine communication channels are not intended for medical emergencies.
If you believe you are experiencing a medical emergency, call 911 immediately or go to the nearest emergency department.
Do not use the website, email, text messages, contact forms, or routine telehealth support channels for urgent or life-threatening symptoms.
5. No Provider-Patient Relationship Through Website Content
General information provided on the website is for educational and informational purposes only. It is not medical advice and should not be used as a substitute for evaluation by a qualified healthcare provider.
Use of the website, completion of a form, communication with support staff, or payment for a service does not by itself create a provider-patient relationship.
A provider-patient relationship is established only after the treating clinical practice accepts you for care through its applicable intake, identity-verification, consent, and clinical-review process.
6. Medical Decisions and Treatment
Healthcare services, prescriptions, laboratory orders, and treatment plans are provided only when a licensed provider determines that they are lawful, medically appropriate, and safe.
Payment does not guarantee:
- Acceptance as a patient
- Approval for treatment
- A prescription or medication
- A particular medication, dosage, brand, or compounded product
- A laboratory order
- Pharmacy dispensing or shipment
- Insurance reimbursement
- A specific clinical, cosmetic, weight-loss, hormonal, or other outcome
The provider may require additional information, laboratory testing, follow-up appointments, medical records, identity verification, or in-person care before providing or continuing treatment.
7. User Accounts and Information
You are responsible for providing accurate, truthful, complete, and current information.
You agree not to:
- Use another person’s identity or account
- Provide false medical, billing, age, identity, or location information
- Hide relevant medical history, medications, symptoms, or diagnoses
- Submit misleading information to obtain a prescription or service
- Allow another person to access your account without authorization
You are responsible for protecting your account credentials and for activity performed through your account. Notify us promptly if you believe your account has been accessed without authorization.
8. Telehealth Services
Telehealth involves the delivery of healthcare services through electronic communication technologies. Telehealth may include video calls, phone calls, online questionnaires, secure messaging, electronic forms, and remote review of medical information.
Telehealth has potential benefits and limitations. Technical failures, interrupted connections, incomplete information, and limitations of remote evaluation may affect the service.
You agree to:
- Provide your correct physical location during each telehealth encounter
- Participate from a private and reasonably secure location when possible
- Provide accurate emergency contact information when requested
- Not record a telehealth visit unless the provider agrees and recording is permitted by law
- Follow instructions regarding laboratory testing, monitoring, medication, and follow-up care
A provider may determine that telehealth is not appropriate and may recommend or require in-person care.
9. Prescriptions, Medications, and Pharmacies
Prescriptions are issued only when a licensed provider determines that a medication is clinically appropriate and legally permitted.
Veloryn Health does not guarantee that a pharmacy will accept, compound, dispense, ship, or refill any prescription.
Medication availability, formulation, pricing, shipping, refill timing, and dispensing are subject to pharmacy policies, applicable laws, supply availability, and provider authorization.
Compounded medications are not reviewed or approved by the U.S. Food and Drug Administration in the same manner as FDA-approved medications. A treating provider should explain relevant risks, benefits, and alternatives when compounded medication is considered.
Pharmacies may maintain separate terms, privacy policies, shipping rules, and refund policies.
10. Laboratory Services
A healthcare provider may require laboratory testing before treatment begins or while treatment continues.
Laboratory services may be provided by independent third parties. Those laboratories may have their own fees, policies, privacy practices, testing procedures, and refund terms.
You are responsible for completing required laboratory testing within the requested timeframe. A provider may delay, modify, pause, or discontinue treatment if required laboratory results are unavailable or incomplete.
11. Payments and Billing
Prices, payment terms, recurring billing terms, cancellation terms, and refund terms will be disclosed before purchase or enrollment where required.
By submitting payment information, you authorize the applicable payment processor to charge the payment method you provide for the selected services and any disclosed recurring charges.
You are responsible for:
- Providing accurate billing information
- Keeping your payment method current
- Paying applicable fees and charges
- Reviewing disclosed pricing and renewal terms before purchase
- Contacting us promptly regarding suspected billing errors
Prices may change for future purchases, renewals, services, pharmacy charges, laboratory charges, or vendor fees. Any applicable change will be disclosed as required by law.
12. Subscriptions and Automatic Renewal
If a membership, subscription, or recurring payment program is offered, the price, billing frequency, renewal period, cancellation method, and applicable refund terms will be disclosed before enrollment.
By enrolling in a recurring program, you authorize charges at the disclosed interval until the subscription is cancelled or otherwise terminated.
Cancellation stops future recurring charges after the effective cancellation date. Cancellation does not automatically refund charges already processed or services already provided.
Automatic renewal and cancellation practices will be administered in accordance with applicable consumer-protection laws.
13. Refunds and Cancellations
Refunds, appointment cancellations, missed appointments, medication charges, laboratory charges, and subscription cancellations are governed by our applicable Refund and Cancellation Policy.
Completed consultations, clinical reviews, administrative work, laboratory reviews, treatment-plan preparation, pharmacy coordination, and other services already performed may be non-refundable to the extent permitted by law.
Prescription and compounded medications are generally not returnable or refundable after they have been compounded, dispensed, shipped, or released by the pharmacy.
14. Communications
By providing your phone number, email address, or other contact information, you consent to receive administrative and service-related communications, including:
- Appointment reminders
- Intake and consent requests
- Billing and payment notifications
- Pharmacy and laboratory updates
- Shipping or fulfillment notifications
- Account and security notices
- Customer support messages
- Changes to services, policies, or legal terms
Communications may be sent by email, telephone, text message, patient portal, or other methods you provide or authorize.
Standard messaging and data rates may apply. Email and text messages may not always be completely secure.
15. Privacy and Medical Information
Your use of the website is also subject to our Privacy Policy.
Protected health information may also be governed by the applicable Notice of Privacy Practices issued by the treating clinical practice.
Veloryn Health may handle protected health information as a business associate of the treating clinical practice where applicable.

17. Intellectual Property
The website and its content, including text, graphics, logos, branding, images, videos, forms, workflows, designs, software, and other materials, are owned by or licensed to Veloryn Health and are protected by intellectual-property laws.

- You violate these Terms
- You provide false, misleading, or fraudulent information
- You misuse the website or services
- Your conduct threatens patient, provider, staff, vendor, or system safety
- Payment obligations remain unresolved
- Continued access would violate applicable law
A healthcare provider may separately pause, modify, or discontinue treatment based on clinical judgment, safety concerns, incomplete monitoring, legal restrictions, patient location, medication availability, or failure to follow care requirements.
24. Governing Law
These Terms are governed by the laws of the State of California, without regard to conflict-of-law principles, except where the law of the patient’s state, healthcare law, privacy law, consumer-protection law, or another mandatory law applies.
Nothing in these Terms limits any non-waivable legal rights available to a patient or consumer.

We may update these Terms periodically to reflect changes in our services, business practices, technology, or legal obligations.
The revised Terms will be posted on this page with an updated effective date. Your continued use of the website after revised Terms become effective constitutes acceptance of those revised Terms, to the extent permitted by law.
